Kingswood Community Center Advisory Board Survey

Hello: Thank you for your service on the Kingswood Advisory Board. We appreciate your concern and dedication to assist Kingwood and those in the community served by the Kingswood Community Center. We have recently hired a new Executive Director, Mr, Anthony Powell. As Mr. Powell begins his work to move the organization forward, we are going through a Strategic Plan process. Your input is valuable to us. You are invited to participate in our survey; The Kingswood Advisory Board Survey. In this survey, each Advisory Board Member will be asked to complete a survey that asks questions about their involvement on the Advisory Board. It will take approximately 2 minutes minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
