Hello: As a mental health institution, we've chosen you to participate in a brief survey regarding a revolutionary mobile application that can possibly advance the way we deal with mental illness. We are LyfeNote, Inc. We seek to improve patient/physician relationship by our application that is essentially a video journal to accurately document how patients are feeling between consultations. This survey is less than 10 questions long and should only take a couple minutes of your time. Your participation in this study is completely voluntary.
