Dear Student!

This short questionnaire forms an important part of my PhD which is titled The Influence of Service Design on Service Innovation in Higher Education. Specifically, I am aiming to test an user innovation concept which I developed over the last year in cooperation with the SCU Library. Thus, the following questions will ask you about your knowledge of library services, your educational pathway, and some general background data. The completion of the questions will take approximately 5 minutes.

Please note that all data collected for the purpose of this study will be aggregated into group form. Therefore none of the information provided will be directly attributed to you. In addition, all data obtained from this questionnaire will be handled strictly confidentially and in a depersonalized manner. This research has been approved by the Human Research Ethics Committee at Southern Cross University with the approval number ECN-12-314.
