You are invited to participate in our survey the increasing incidents of suicides within the ranks of the JCF. In this survey, approximately 200 people will be asked to complete a survey that asks questions about Suicidal thoughts and tendencies among members of the JCF. It will take approximately 15 minutes to complete the questionnaire.
1. This questionnaire is a survey tool being used as a part of a research project.
3. Please be advised that all information gathered will remain anonymous and will be
treated with the strictest of confidence. Information will be used only for the
purpose of this research.
4. Please do NOT write you name on this questionnaire
5. Your participation in this survey is absolutely voluntary
6. Please answer all questions honestly and accurately By participating in the survey, I understand that: 1. My anonymity will be preserved at all times because my name and any other identifying information
will be kept strictly confidential and the results of this study will be reported in such a manner that I will not be identifiable in any way.
2. My participation in this study is voluntary and I reserve the right to refuse to answer any question I
so desire.
3. I can discontinue participation in this study at any time, for any reason without fear of any negative consequences to me.
4. I will not be compensated for my participation.
