You are cordially invited to participate in a survey by the West Maui Improvement Foundation on health care needs in West Maui and througout the island.

. Your participation is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Any quotations you provide may appear on the hospital web site, westmaui.org, or other published materials bunedited except for inclusion of inflammatory commentary and no names will be provided. A complete list of survey responses by others can be found at westmauihospital.or

Your survey responses will be strictly confidential and data from the research multiple choice questions will be reported in the aggregate. Your individual comments will be recorded as well but no source will be identified. Your information will be coded and will remain confidential. Proprietary software will allow you to complete the survey only once.
