Survey to Determine Factors that Affect Safety

My name is Bill Bannister. I currently work for BP Pipelines (North America), Inc. in the Operations Excellence Training Group and am a student at Purdue's North Central Campus. My fellow students & I are working with Sarah Sanders-Smith, a Professor at Purdue University. She is the Director - of Purdue North Central's Center for OSH and a published author in the American Society of Safety Engineers' (ASSE) "Professional Safety" Journal.

We are conducting a study to develop a Safety Program Risk Matrix. It is anticipated that this matrix will allow companies to compare their safety programs (annual spend on safety, hours of training/year, etc.) with the safety programs of their peers to determine what adjustments, if any, could be made to a company's safety program to improve on TRIR, LTIR and EMR. It should take approximately 20 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions so that we can provide shared learning's to a broad industry group. This study could be used by many companies as a benchmarking tool.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
