Dear Parents,

It is important that we hear from each family who has withdrawn their child(ren) from Green Meadow. We strive for open communication and mutual understanding with our families and would ask that you fill out this questionnaire with the option of requesting a meeting with an Administrator (or another person of your choice). We are interested in your experience and your child(ren)'s experience and would appreciate any suggestions or recommendations you might have. We thank you for taking the time to communicate with us about ways we could improve our school.
