In order to effectively and efficiently manage future upgrades, it is vital that we receive your honest feedback on this most recent endeavor in order to improve the overall management of the Facets Upgrade projects. A summary of the responses will be reviewed and discussed as a project team.

The questions in this survey cover all phases of the project. All feedback will be completely confidential.
