You are being invited to participate in our Healthy Shake-Away survey being conducted in the North-Side area of Belize City.

This study is being conducted as part of the requirements for an undergraduate degree at the University of Belize. The questionnaire will take approximately fifteen (15) minutes to complete. Please note that your participation in this study is totally voluntary and your identity will remain anonymous at all times.
Steps have been taken to safeguard your privacy. In case the data collected in this study will be published, your identity will not be revealed.

You are invited to participate in my survey on Smoothies. In this survey, approximately 100 people will be asked to complete a survey that asks questions about their drinking habits. It will take approximately 5 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
