The purpose of this form is to provide an easy way to report your progress on a weekly basis. The form can be filled out and submitted multiple times - a blank form will appear each time you click the link. A link to the form can be saved in your Internet browser favorites or found by searching your email inbox containing the link. It can also be completed on a smartphone, if you have access to one. Each of the Army Values are applicable here as this is a self-assessment. By making the effort to follow your individual plan, and reporting that progress, you can and will make the improvements necessary to pass the APFT and/or change your body composition in order to comply with the Army's standards and improve your lifestyle fitness.

For "UP," you met or exceeded close to all goals for the week (75-100%+). For "NEUTRAL," you had an "average" week (50-75%), and "DOWN" means you didn't come close to hitting your goals, need improvement, were unable to work out, were injured, etc. (0-50%). No one will always have an "UP" week - the intent is to track progress and encourage effort towards meeting your goals. Strive for as many neutral and up weeks as possible for consistent improvement. For further guidance on how to self-assess, contact your LFP Coordinator.
