This survey has a dual purpose:
1. It helps us identify the business application(s) you are using to perform your work and to rate the importance of that application.
2. It also allows us to obtain a general overview of each application by identifying those that exhibit or perform the same business process functions including any gaps that may exist between them.
This study includes the following:
1. The name of the business application
2. The region of implementation (AMERICAS, APAC, EMEA)
3. Office/Plant covered
4. The name of the Customer Champion
5. The name(s) of the Key User(s)
Your survey participation has been recommended by your Global Quality leaders in the organization to help in this process. It will take approximately 10 to 15 minutes to complete the questionnaire. A. CORE: Without this application, the business function is unable to operate; no workarounds are available
B. CRITICAL: Without this application, the business cannot operate efficiently and will be less profitable. However, workarounds are available (ex: if email is down, phone or fax can be used)
C. IMPORTANT: in the event of a service disruption, the business processes could be seriously impaired; however, the business would still be able to function. Production will surely decrease, but not necessarily profitability.
D. USEFUL: These services pose minimal threats to neither profitability nor productivity in the event of a service disruption
