Practicum Evaluation

Hello:
We are interested in evaluating your practicum experience at MHDR so that we can determine what we are doing well and how we might improve what we're doing now. We would much appreciate if you would join your fellow practicum students at MHDR in this effort by responding to this survey. We have tried to keep it brief but invite any additional comments you may way to make, either her or in person to your supervisor(s).

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you should feel free to skip them. However, it would be very helpful and important for us to learn your opinions.

Your survey responses will be confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
