Thank you for registering to participate in the SERVIR Collaboration Forum, to be held May 18-19th, 2015. We look forward to learning more about your programs, and to exploring potential opportunities for collaboration with SERVIR – as well between other programs - that can help strengthen our collective efforts to improve climate-resilient decision-making in developing countries.

In preparation for the SERVIR Collaboration Forum, we are conducting a pre-forum survey so we can make the most of the event. The survey has two purposes:
- Collect information about the participants’ expectations of the Forum to help shape the agenda;
- Gather systematic information about the programs you represent. This information will provide a starting point for discussing current and future collaboration opportunities and to help “map” existing connections.

Since one of the purposes of the survey is to learn more about specific programs, participants are divided into two categories:
1) Participants who are representing their institutions/departments more broadly (vs. a specific program);
2) Participants who are representing a specific program implemented by an institution.

NOTE: if you are participating at the Forum representing more than one program, we would appreciate if you could fill out (or delegate someone to fill out) the survey for each program represented.

This survey is confidential. However, please note that all information generated by the survey and the Collaboration Forum itself will be used and shared with participants in aggregate.
