Exit Survey

Dear ANS Member - As you know, as a benefit of your ANS membership you receive a monthly subscription to the American Nuclear Society's Nuclear News. We'd like to invite you -- and encourage you -- to participate in this brief survey about Nuclear News. It will take approximately 10 minutes to complete the questionnaire.

Your participation in this study is completely voluntary, although we value your input as we endeavor to make Nuclear News even better, and maintain its reputation as the world's premiere magazine for nuclear professionals, educators, researchers and students. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. PLEASE NOTE: After the first 7 questions, which are radio button "select" style questions, you will have an opportunity to provide written comments in comment boxes for questions 8 and 9.

It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. If you have questions at any time about the survey or the procedures, you may contact John Smith in our Membership Department at 123-456-7891 or via email at jsmith@ans.org.
We thank you very much for your time and support. Please start with the survey now by clicking on the Continue button below.
