Evaluation Activities Log

This is the event log for the Building Healthy Communities (BHC) program evaluation. Each month SCUSD is required to submit a list of activities and events to LPC Consulting Associates to support the BHC evaluation. Use this web survey to record your activities/events that are aligned with BHC initiatives.

Guidelines:

(1) Use this form to complete data collection activities. Complete this form for every individual activity/event.

(2) Be prepared to enter the following key information:

A. A brief description of the activity/event, including the name of the event/activity
B. The time and location of the activity/event
C. The number of participants, including their age and ethnicity
D. Whether this is the first time this event has occurred
E. Other organizations that were involved in the activity/event
F. Information on any media coverage (if applicable)

(3) It is perfectly acceptable to estimate or "guestimate" if you do not have all the information. The purpose of this data collection activity is to get a sense of the impact of activities and events on the community, not to create a "scientific" study or data collection activity.
