As a Washburn University Master Social Work student, this Needs Assessment Survey is a required assignment in the SW 692 Generalist Seminar I course I’m enrolled in for Spring 2016. The sole purpose is to help me learn how to develop and distribute (practice only) an agency needs assessment. In accordance with the NASW Code of Ethics 3.09 Commitment to Employers (b) social workers should work to improve employing agencies’ policies and procedures and the efficiency and effectiveness of their services (NASW Code of Ethics 2008).

The purpose of this Needs Assessment survey is to gather information for determining gaps and/or needs of agency/program improvement(s) that would better serve staff and the clients we work with. In completing this assignment, you will be helping me learn what works well in our agency, as well as areas you believe may be improved.
