INTRODUCTION Greetings! You are invited to take part in this academic research. Please take time to carefully read the information below and decide whether or not you wish to take part in this study.
ABOUT THE STUDY. This study aims to examine charity giving attitudes and behaviors. The survey is divided into 3 main sections. Part 1 asks questions about personal charity giving practices, preferences, and motivations. Part 2 asks questions about your personal awareness and charity giving attitudes about two social issues. Part 3 asks about your personal awareness and charity giving attitudes about a specific charity organization. Note that there are NO RIGHT OR WRONG ANSWERS. Please also bear in mind that this is an academic research study and is NOT independently commissioned by any charity organization, local or international.
VOLUNTARY PARTICIPATION. Participants are preferred to have the following characteristics to answer this survey: — 25 years old and above — Permanent resident in the Philippines Taking part in this survey is completely voluntary. If you decide to take part, you are still free to withdraw at any time without any consequence to you. However, your completed response to the survey will be most valuable.
CONFIDENTIALITY AND ANONYMITY. Be assured that all answers you provide will be kept in the strictest confidentiality. While several demographic information will be requested at the end of the survey, you will not be required to provide any identifying information (i.e., your name). As such, your answers will be completely anonymous.
RISKS AND DISADVANTAGES. The questionnaire will take approximately 30-45 minutes to complete, therefore this study will take up some of your time. But apart from that, taking part in this study involves no other risks.
RESULTS. The results will be written up as part of the culminating project of the principal investigator for her graduate degree.

Charity giving is the voluntary giving of money, goods, or time to help the unfortunate either directly or indirectly. In this study, however, note that we EXCLUDE direct almsgiving and religious offerings or tithings.
