The purpose of this questionnaire is to collect information from all Quebec EDs to inform the development of an “Elder-friendly ED” assessment tool. This tool would be used by EDs to assess the quality of the care and services offered to older patients.

This questionnaire contains the following sections:

Section A: “Elder friendly” ED services. This section asks about the care and services for older patients at your ED. It also asks your opinion about which aspects are the most important and should be retained in an “Elder-friendly ED” assessment tool

Section B: Your opinions - about the care offered at your ED.
Section C: Background - about you and your ED

The questionnaire should take about 15 minutes for you to complete.

SECTION A: ELDER FRIENDLY ED SERVICES
 
 
This section covers the physical and human environment and services you have in your ED for older adults. We would like your feedback about which of the questions should be included in final version of the “Elder-friendly ED” assessment tool. After groups of questions, you are asked whether these questions items should be included in the final tool by checking one of the following boxes:

A) Retain as essential or core questions for the final tool
B) Optional or complementary questions for the final tool
C) Discard from the final tool

Please bear in mind that we are asking both the ED chief physician and the ED head nurse or nurse manager to fill out this questionnaire . Please answer the following questions to the best of your ability. Feel free to ask a member of your staff to help. If you don’t know the answer to any of the questions please “check” don’t know (DK).
