This is an example of Presentation text, with the setting changed to "Header"

This is Presentation Text with a setting of "Sub-heading"

Note the absence of a separator between the previous text and this text - this is removed by clicking on the separator option on the RHS.
This is a Section header with the setting changed to "Default".

This is Presentation Text with an "I agree" check box option.
Branching logic allows you to terminate the survey, or go to the "Thank you" page, as well as using normal branching to other questions in the survey.

A page break can be inserted by clicking on the "Page Break" on the RHS.
The break is inserted after the question where the "Page Break" option is used.

This is a Comment Box. Options are single row text, multiple row text, email address, or numeric input.

Validation can be required - a response is then required.

Note that when a carriage return is used, a new input box is created.

You can include instructions for the question via the Settings option. This has Alternating colours with Border, and default pixels.

This is a Multiple Choice (select one) question.
You can have as many answer options as you want.
The Other box is optional, selected when creating the question.
The options have been spread over two columns - this number can be chosen via Settings

This is another Multiple Choice (Select one) but with drop down lists.

A Footer can be created simply by clicking on the "Edit Footer" icon at the bottom of the page. Note that the footer applies to every page.
