Instructions & the Rating System

Instructions
1. Rating from 1-5: This will be used to rate the new hire for that day. An overall rating is not required, though it is important to follow the guidelines below for additional insight on how to properly rate an employee’s performance.
2. Remarks and Comments: This section is strongly encouraged. If there is no additional feedback to be given or it is not applicable, please write “N/A.”
3. Training Specific Questions: Basic yes/no questions to make sure that you were put in a position where you felt you could train to the best of your ability.
4. Additional Comments: Any and all additional feedback is appreciated in this process. Remember, these evaluation forms are to help the new hire find their place in the company where they will not only succeed in, but enjoy as well.
5. Signature: Please sign the document at the end.
6. Submit: Please drop off the completed evaluation form to either the Training Specialist or current Manager involved with training.

How to Rate
5. Outstanding: Performance far exceeds expectations. Demonstrates an unusually high level of performance and has distinguished themselves in this respective field.
4. Exceeds Expectations: Provided a successful demonstration of the field given and has often exceeded those requirements in more than one area.
3. Meets Expectations: Employee met the requirements in this field.
2. Needs Improvement: Small errors prohibited the employee from meeting the required expectations. In time, however, they are certainly capable of improving these skills.
1. Unacceptable: Represents a rating that would not work well within the established Jet environment.

For reference: This form is as much about prevention as it is evaluation. The goal here is to help employees reach a place where they are comfortable after training, performing well, have a strong potential for growth, and will be a welcome addition to the team. Answers will be subjective; and that is okay! These evaluations are meant to improve performance, not to blame.
