Thank you for your participation in America's Health Insurance Plans (AHIP) Survey on Improving Patient Safety and Appropriate Use of Healthcare Services.

AHIP is conducting a survey of its members to gain a better understanding of health plan efforts to promote patient safety and appropriate care. Data collected through this survey will be used to develop issue briefs and inform policy makers and the media about health plans' efforts in this area.

Your individual responses to the survey questions will be strictly confidential. Summary information based on the aggregate responses of all participating plans will be reported.

Please answer all of the survey questions based on your health plan’s commercial products. Plan initiatives and programs aimed at Medicaid enrollees and Medicare Advantage beneficiaries are outside the scope of this survey.
