This survey is designed to generate a clearer understanding of the nature and scope of Ethics Committees in SCLHS facilities. This is a learning survey; this survey is designed to learn from each of the local hospitals, and for us to learn from each other. The intent of the survey is to pull together various understandings from throughout the ethics committees and resource groups throughout the system. The results will facilitate mutual learning.

Through this survey we hope to gather general information about the Ethics Committees, how the Ethics Committees function, how persons are selected to be on the Ethics Committee, level of training of members (actual/desired), the scope of authority for the Ethics Committee, and the like.

The benefits of completing this survey are:
1. Each person has the opportunity to reflect on and share their experience of their Ethics Committee.
2. Each person is afforded a space to share her or his thoughts regarding Ethics Committees.
3. Summary and aggregate data will be developed and distributed, thereby creating data for future planning and development at both the local and system level.
4. Outcomes will help to generate a knowledge base in order to create consistency and a learning exchange within and among SCLHS facilities.
5. Outcomes will be used to learn and establish evaluation instruments in the future.
