You have been identified as a CDC subject matter expert (SME) who has been involved in developing the 15 core public health capabilities under the new preparedness program framework; and who may also provide training/technical assistance to Public Health and Emergency Preparedness (PHEP) awardees. Therefore, DSLR would like to request your participation in the PHEP program training and implementation needs assessment to better inform the design and development of training and implementation support for the 2011-2016 PHEP cooperative agreement.

The purpose of the survey is to assist DSLR to better identify:
1) training needs for awardees and CDC staff related to the PHEP program from your perspective
2) existing training opportunities related to preparedness in your subject matter expertise

The questions in this section are intended to improve future preparedness training for PHEP awardees by identifying:

1) Potential gaps as it relates to the 15 PHEP capabilities
2) Existing training opportunities your agency/division/branch currently offers that PHEP awardees can potentially access.

Please consider “training” to be formal training (e.g. face-to-face classroom, video presentation, webinar, video conference) as opposed to one-on-one technical assistance.
