You are invited to participate in a survey to gather information on the awareness of the Epilepsy Foundation of South Carolina and its services as well as Riverfest, its major fundraiser. This is part of my senior class project at the University of South Carolina. This survey will take approximately 5 minutes to complete.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.
Your survey responses will be stricly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. Have you or someone you know received service from the EFSC? If the answer to question 3 was no, skip to question 18. In which way was the service received from the EFSC? What type of service (s) would you like to be offered through the EFSC? Was a referral to a support group or phone contact requested from EFSC? If a referral to a medical or community resource was received by the EFSC, was the resource contacted that was suggested? Was EFSC service easy to use?
