Hello again,

Now that the 20 week PEPEN Pilot program has conlcuded, you are invited to participate in our final survey. In this survey, approximately 10 people will be asked to complete a survey that asks questions about the effectiveness of the PEPEN Pilot 2003.

It will take approximately 10 minutes to complete the survey, and a stamped, self addressed envelope has been provided for you to return your completed survey to the Project Officer.


It is very important for us to learn your opinions, although your participation in this study is completely voluntary. If you feel uncomfortable answering any questions, you can withdraw from the survey at any point.


Your survey responses will be strictly confidential, and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
