Hello: Nutrition and Physical Education Directors


We are requesting your assistance in conducting a follow-up study to a 1992 investigation done for the CDC detailing the Status of Nutrition Education at the State Level. This survey has been distributed to each state and respondents will be asked a number of questions regarding the policies surrounding nutrition and physical education in each particular state. It will take approximately 10-15 minutes to complete the questionnaire.


Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to gain information from you in order to compile an accurate follow-up, however, so your participation is extremely appreciated.
