I am a senior in the Social Science program at Chapman University. As part of the requirements for graduation, I am conducting a survey regarding postpartum depression in Yuba and Sutter counties. I intend to ask 80 women to complete the questionnaire. It will take about 10 minutes of your time.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point.
Your survey responses will be stricly confidential and any information you choose to provide will be coded so as to be indiviually unidentifiable, and will remain completely anonymous.
