INFORMATION SHEET

You are being invited to take part in a research study. Before you decide, it is important for you to understand why the research is being done and what it will involve. Please take time to read the following information carefully and discuss it with others if you wish. Contact me if there is anything that is not clear or if you would like more information. Take time to decide whether or not you wish to take part.

What is the purpose of the study?
My name is Kirsty Guthrie, I am an undergraduate student at Glasgow Caledonian University, and I am conducting this study to fulfill the requirements of my degree. The purpose of the study is to investigate the perceptions of deaf individual’s about what they think are essential elements that must be included in order for a counselling session to be successful. The research will investigate variables such as; communication method used in the counseling session, the counsellor’s knowledge and experience with the deaf community, counsellor’s training and counseling qualification on the perceived success of a counseling session.

Why have I been chosen?
You have been asked to take part because you fit the criteria of this study. This criteria is that participants of the study are required to identify themselves as being deaf, are over the age of 18 and are not currently in any medical programmes for the treatment of deafness.

Do I have to take part?
No. It is up to you to decide whether or not to take part. If you do decide to take part you will be given this information sheet to keep and be asked to provide consent. If you decide to take part you are still free to withdraw at any time and without giving a reason. A decision to withdraw at any time, or a decision not to take part, will not affect you in any way.

What will happen to me if I take part?
If you consent to taking part, you will be asked to complete an online questionnaire via Survey Monkey. This questionnaire will require you to give some information such as sex, age and country of residence. You will then be asked to read a few short counselor descriptions and answer a few questions based on each of these descriptions. After this there will be a few open ended questions in which you will be required to type in a brief answer. This survey should take no longer than 30 minutes to complete. All answers to questions will be confidential and you will not be required to give any information that would reveal your identity.
Will the information I provide be kept confidential?
Yes. No one but myself will have access to any information that could identify you. You will not be asked to reveal any information that could identify you throughout the questionnaire and no personal details such as email addresses will be obtained by myself the researcher.

What will happen to the results?
The results will be written up in my final project which will be completed by the end of March 2013. It is necessary for me to retain original data until March 2014(?) After that, all questionnaires will be destroyed.

Who is supervising the research?
My supervisor is Dr Elaine Duncan. She will be happy to provide further information on any aspects of the research if necessary.
