I understand that I am being asked to participate in a survey activity that forms part of Sarmeyah Kugadasan (student’s name) required dissertation. It is my understanding that this survey has been designed to gather information about the following subjects or topics:

- Obesity epidemic
- Readymade smoothies and smoothie providers
- Ethical issues
