What was the reason for your last visit to your FAMILY/GENERAL PRACTICE physician? A Wellness Exam may include checking your blood pressure, performing a body fat test, checking cholesterol and levels of good and bad fats in your body and comparing this information to data for someone of similar age, sex and other variables. When considering an annual physical/wellness exam, please comment on the following: I am diligent about having one every year I don't have a wellness exam every year, but I would like to have one available to me I would like to have a wellness exam every year, but I am too busy and getting to see my doctor is too inconvenient during the work week I would like to have have access to the information provided by a wellness exam, but I don't like doctors or nurses My doctor has never offered such a service to me Please note, some of the following questions are targeted towards individuals who are currently employed. If you are a full-time student or this does not apply, please use your last place of employment as a reference for these questions. My company puts on a health fair every year and a wellness exam is available to me then I see the value of a wellness exam If you would like to have a wellness exam every year, please comment on the following: Having to get a blood sample drawn would be enough to turn me off having a wellness exam If it was possible to have a wellness exam performed in a private environment at my work location, I would be much more inclined to have one I would rather pay for this service out of pocket rather than through insurance because I don't want the insurance company knowing too much about me A flexible spending account is a service offered by employers so that employees can use pre-tax dollars to pay for uncovered medical expenses such as co-pays, flu shots and eyeglasses. Assuming I have a flexible spending account with my employer, I would like to use these pre-tax dollars to pay for this service rather than pay for it out of pocket. Recall from earlier that a Wellness Exam may include checking your blood pressure, performing a body fat test, checking cholesterol and levels of good and bad fats in your body and comparing this information to data for someone of similar age, sex and other variables. Also keeping in mind that
the co-pay for a visit to your family doctor is about $10, depending on whether you have a HMO or PPO insurance, having laboratory tests performed can add additional out of pocket costs, time away from work or family is required to visit your doctor
