Administration Staff Pre-Visit Communication Questionnaire

Hello: The focus of this project is how we can improve patient communications in this practice. That is, how we can assure that patients can communicate their dental concerns and other potentially-related medical issues to administration staff, hygienists, assistants and dentists. By May 21, 2015 I’d like to get a sense of your concerns and perceptions of how patient communications is currently being conducted. This will help me to get a better understanding of your practice before my workshop with you. Your responses are anonymous and confidential and will be only used to help me understand communication issues before I arrive. I am looking forward to visiting with you in May 27, 2015.
