CONSENT TO PARTICIPATE IN RESEARCH Prius and Plug-in Hybrid Vehicle Driver Study
Introduction Susan Shaheen Ph.D., Tim Lipman, Ph.D, and Rachel Finson, researchers at the Transportation Sustainability Research Center (TSRC) at the University of California at Berkeley invite you to take part in the study Prius and Plug-in Hybrid Electric Vehicle Driver Evaluation.
You are being invited to participate in this study because you responded to the recruitment email sent out by your employer or building management, you meet all of the requirements to join the study and have filled out a Study Application and agreed to adhere to the vehicle restrictions outlined in the Study Application and the Driver Consent.
Purpose The purpose of the study is to better understand average drivers' experiences with the two study vehicles, the gasoline-electric hybrid Prius and the plug-in hybrid vehicle. To do this we will gather information on participant perceptions, criticisms, comparisons, and possible enhancements regarding the vehicles. Driver data will also be collected automatically to better understand the energy and environmental impacts of the two study vehicles. Researchers will use all of this information to make recommendations on plans for future vehicle designs and early placement of PHVs with consumers.
Procedures As a participant in this study you will be driving two study vehicles, a conventional gasoline-electric hybrid Prius, and a Plug-In Hybrid Vehicle (PHV). Permission and consent to conditions for driving the study vehicles is provided in the Motor Vehicle Consent and Agreement (“Driver Agreement”) between yourself and the University. The Driver Agreement states the conditions and vehicle use restrictions for driving the study vehicles and the insurance that is being provided.
While you are participating in the study, you will:
• Fill out a series of on-line surveys at two-week intervals (five surveys total), and; • Participate in a group interview at the close of your participation in the study.
Operational data will be automatically, continually recorded on the car's computer system, and will be transmitted to Toyota and UCB project staff at periodic intervals. The data will be transmitted wirelessly and remotely and/or stored on an in-vehicle data card that will be downloaded by project staff immediately after you have completed the four-week driving period for each vehicle. This data will include vehicle speed, GPS, trip length, trip date and time, charging time and duration, battery system energy use, fuel use per trip, battery state of charge, and electric vs. engine operation.
Study Time: As a participant in this study, you will be asked to drive each vehicle for four weeks (eight weeks total). You will drive each vehicle instead of your personal vehicle as often as possible during your participation in the study. An on-line survey will be administered at two-week intervals to record your responses and experiences to each of the study vehicles. Each survey will take 10-15 minutes to complete. The group interview/focus group will be conducted at the close of your participation in the study and will take two hours.
Study Location: For any interaction regarding the study vehicles, research staff will meet you at your place of employment or residence where you initially received the recruitment email. You may take the surveys from any Internet connected computer. The group interview/focus group will be at a location convenient to your place of employment or residence where you initially received the recruitment email.
The manufacturer of the study vehicles is providing 24-hour roadside service. As noted in the Driver Agreement, only roadside service companies approved by the manufacturer may be used for the study vehicles. The study vehicles may not be driven outside the State of California.
Benefits Although no personal benefits are anticipated from your participation in this research, the results of the study will provide researchers with an in-depth understanding of the two study vehicles. The results of this research may lead to new automobile design features and technologies.
Risks/Discomforts • Some of the survey questions may make you feel uncomfortable. You do not need to respond to any survey questions that make you feel uncomfortable. • Some of the group interview/focus group questions may make you feel uncomfortable. You do not need to respond to any questions asked during the group interview/focus group that make you feel uncomfortable. • You can discontinue participation in the study at any time with no consequence to you. If you discontinue participation in the study, for any reason, you must promptly return the study vehicle that is in your possession at that time. • You may find that plugging in a car for fuel makes you feel uncomfortable. If you feel uncomfortable you may request further training from the project staff. If you continue to feel uncomfortable you can discontinue participation in the study with no consequence to you.
Confidentiality The surveys will be anonymous and your answers will not be connected to you in any way. Answers you provide during the group interview/focus group will be recorded to ensure accurate summaries of the group’s experience with the vehicles, but will not be connected to you in any way. The audio recording will be destroyed at the end of the study. The data from the gasoline receipts that you provide and the electricity bills will be recorded and maintained separately from your personal information. All data will be encrypted on hard drives using TrueCrypt.
All of the information that we obtain from you during the research will be kept confidential. We will store any audio recording, notes, and surveys in a locked file or a secure computer. Your name and other identifying information will not be used in any reports of the research. The audio recordings will be destroyed after they have been transcribed and the summaries are complete. After this research is completed, the transcribed audio data, notes and surveys may be saved for use in future research by University of California and other alternative fuel deployment researchers. However, the same measures to protect confidentiality described above will apply to future storage and use of the materials.
Every effort has been made to protect the anonymity of the research participants. One research staff member will manage the names and identities of the participants. Another research staff member will manage the data. Toyota will have access to the data, but will not have access to driver identification. Under extenuating circumstances (out of control of the research staff, such as under subpoena during a legal proceeding) the identity of the research participants could be revealed to Toyota or legal authorities and could be linked to the data.
Please note that the Driver Agreement form contains a number of specific requirements regarding confidentiality and disclosure of information related to your participation in the study. Be sure to read these detailed requirements in the Driver Agreement.
Compensation Study participation involves considerable investment of time and possible inconvenience with no compensation offered. You will be responsible for fueling costs (gasoline and electricity) for the Prius and the PHV.
Treatment and Compensation for Injury The manufacturer of the study vehicles is providing automobile liability insurance of three million dollars ($3,000,000) with combined single limit per occurrence for bodily injury and property damage with respect to the vehicles. However, as noted in the Driver Agreement, certain limitations apply: 1) coverage for subject injury will not extend to intentional conduct, willful misconduct or gross negligence on the subject's part; and 2) repair of vehicle damage must be covered by the subject if the damage is caused by gross negligence or intentional misconduct of the subject.
It is important that you promptly tell the researcher, Rachel Finson, if you believe that you have been injured because of taking part in this study. You can tell the researcher in person or call her at 510.725.7609.
If you are injured as a result of taking part in this study, University of California will provide necessary medical treatment. The costs of the treatment may be billed to your insurer just like other medical costs, or covered by the University of California or the study sponsor, Toyota Motor Corporation, depending upon a number of factors. Rights Please remember that your participation in this research is voluntary. You may refuse to answer any questions and may stop taking part in the study at any time. However, if you decide not to fill out each survey at the two-week intervals, you will not be able to continue driving the study vehicles.
