Ampegy would like to gain a clear understanding of the reasons or experiences that result in an Indendent Representative cancelling their agreement.

You recently decided to cancel your Independent Representative Agreement with Ampegy. We understand that this opportunity is not the best fit for everyone. We invite you to complete a survey so that you can share your opinions about your experience and help us better understand the reasons behind your decision.

The goal of this survey is to assist us in improving our processes and communications to help ensure that current and future Independent Representatives have a consistently satisfactory experience with Ampegy.
