You are invited to participate in the assessment of CAPTE's Onsite-Review Team. It will take approximately 5 minutes to complete and your participation is entirely voluntary.

This survey is intended for Institutional Administrators, Program Directors, Academic Administrators, and each faculty member who: 1) Contributed to the program's Self-study Report, 2) Participated in the on-site visit, and/or 3) is familiar with the education program and self-study material.

Your candid responses will be used to provide feedback to each on-site reviewer and to guide us in the selection, orientation, and training of on-site reviewers.
