You are invited to participate in our survey Cape Dorset Community Health Check. It will take approximately 5-10 minutes to complete the questionnaire.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. It is very important for us to learn your opinions. You will be asked to rate from 0 (low) to 10 (High) 4 general areas of the Community based on your personal experience and opinion. And there are a couple of questions (4 and 7) that require a written response.
