Hello!  As the nation's largest minority — comprising almost 50 million individuals— people with disabilities contribute to diversity yet there are few research studies in scholarly journals about the workplace needs of people with disabilities.  In fact, many articles about workplace diversity exclude disabilities. I am seeking people with a disability or disabilities to answer 17 questions about their current or previous experiences in the workplace. This information will be used in my graduate research study designed to help people with disabilities have better workplace experiences. Your identity and answers are confidential. It will take 5-10 minutes to complete.

Your participation is completely voluntary  There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important to learn your opinions because the research about this subject is extremely limited. 

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.  The thank you reward for people who submit responses to this survey by March 8, 2015 - a gift card drawing with good odds: three (3) random winners of $30 Amazon Gift cards. For people completing this survey after March 8, 2015, there will be one thank you drawing for one (1) $25 Amazon gift card. As this survey get circulated further the thank you gifts may grow again.
