You are invited to participate in a gathering of information about the understanding of the difference aspects and procedures associated with the service the Coach House Trust is providing.

This survey has been sent to all staff within the Coach House Trust. Depending on the department within the Coach House Trust you work in, you may not know much about a given question - this is OK! Please give as much information as you know but it is also help if you simply answer "I don't know".

It will take approximately 15 minutes to complete the questionnaire however it may take longer if your answers are detailed.
