Dear colleague,

You are invited to participate in this survey on the Awareness and Management of Fatigue: A case study of Helicopters Operations Worldwide.
In this survey, a number of helicopter pilots will be asked questions about fatigue: their understanding, awareness and management of this issue. It will take approximately 10 Minutes to complete the questionnaire.

Your participation in this study is completely voluntary and anonymous. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for me to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. If you have questions at any time about the survey or the procedures, you may contact Capt. A. Ndule at +2348063192774 or by email at [email protected].

Thank you for making it this far. My project dissertation aims to draw industry attention to an acute problem of fatigue, especially the reluctance of crew members to speak up for fear of persecution. While crew members are ultimately responsible to ensure they are in good condition to fly, company resources can be directed at improving the quality of life of these well trained crew to improve productivity and increase safety. Moving forward should more exploration be needed, I may send out a more specific survey to investigate areas of interest among different helicopter operations as I gather and process your feedback. Thank you once again
