1. Inadequately recognizes clinical scenario and frequently unable to describe relevant anatomy and physiology.
 
2. Generally adequate knowledge, but has gaps which makes recognition and treatment variable.
 
3. Base of knowledge covers conditions and knowledge is sufficient to relate clinically.
 
4. Consistently in-depth knowledge in all areas including some uncommon or little known areas.

Data Gathering: History and Physical Exam 
 
1. Routinely incomplete or inaccurate in reporting history at rounds. Depends on MO intern to present.
 
2. Generally complete but sometimes misses essential information or misinterprets findings. Relies on patient file to present hospital course during rounds.
 
3. Complete, accurate and systematic. Asks appropriate questions of patient and detects and understands important clinical signs.
 
4. Is able to summarize a complicated patient’s course without the file, including relevant clinical, laboratory and imaging findings over the hospitalization.
 
5. Equivalent to an excellent consultant or expert in the area- this level is rarely seen.

Clinical Reasoning, Management and Judgment
 
1. Orders investigations blindly and doesn't follow clinical or investigative results, often delaying the care of the patient.
 
2. Sometimes lacks clinical judgment. Follows clinical course but is not timely in following up or making a new management plan.
 
3. Follows patient course and documents appropriately. Seeks senior and/or consultant review appropriately.
 
4. Consistently interprets clinical findings accurately and appropriately. Takes initiative in arranging consultations to other departments or the ICU.
 
5. Equivalent to an excellent consultant or expert in the area- this level is rarely seen.

Diagnostic/Therapeutic Planning 
 
1. Relies on personal experience in guiding patient management and rarely makes use of protocols in patient management.
 
2. Does not consistently demonstrate a logical rationale or practical plan.
 
3. Consistently uses protocols and is able to point out when a protocol is inappropriately being ignored
 
4. Consistently uses protocols and is able to articulate when and why deviation from the protocol is necessary.

Procedural Skills in Obstetrics and Surgery
 
1. Unable to carry out procedures appropriate for their level of training and/or puts patients at harm.
 
2. Able to carry out procedures appropriate for their level of training, but only with detailed instruction.
 
3. Technically competent in the performance of procedures appropriate for their level of training.
 
4. Precise and careful, with good attention to detail. Returns equipment in good condition for another registrar’s use.
 
5. Equivalent to an excellent consultant or expert in the area- this level rarely seen.

Performance under stress
 
1. Unable to recognize urgency of situation or disappears/is unavailable.
 
2. Delays in acting appropriately in an urgent situation and is not able to take initiative.
 
3. Recognizes an urgent situation and can follow instructions given to perform with skill and competence in stressful situations.
 
4. Is confident in performing duties under stress. Shows leadership in delegating tasks and mobilizing resources efficiently and effectively.
 
5. Can anticipate a potentially emergent situation and is prepared to act. Equivalent to an excellent consultant or expert in the area- this level rarely seen.
 
 
 
GOAL 2: COMMUNICATOR
Physician/Patient Relationship
 
1. Is inappropriate in their verbal and body language with a patient and/or their family.
 
2. Uncomfortable communicating with patient and/or family. Communication lacks clarity and is unfocused. Relies on Mental Health counselor.
 
3. Develops rapport with patients but does not listen to or encourage the patient to express themselves.
 
4. Develops strong relationship with the patient and family. Is empathetic, listens and articulates well and puts the patient at ease.
 
5. The patient and family know the registrar’s name. Equivalent to an excellent consultant or expert in the area- this level rarely seen.
 
 
 
GOAL 2: COMMUNICATOR
Communicating management plan with patient
 
1. Patient under this registrar is unable to explain their condition or reason for the procedure.
 
2. Depends on others to communicate management plan to the patient.
 
3. Upon direction, is able to explain management plan or sensitive news regarding diagnosis and prognosis with a patient.
 
4. Is comfortable raising sensitive news regarding diagnosis and prognosis with a patient. Shows empathy.
 
5. Plans and organizes family meetings and is at ease discus sing sensitive news with the patient and her family. Equivalent to an excellent consultant or expert in the area- this level is rarely seen.
 
 
 
GOAL 2: COMMUNICATOR
Written Skills and Record Keeping
 
1. Notes lack organization and clarity and are late, incomplete or inaccurate.
 
2. Notes are generally legible and accurate but disorganized and not always up to date. Sometimes needs reminders to complete notes.
 
3. Notes are complete, accurate and easily understood and completed by deadline.
 
4. Notes are legible, succinct, well-written reports completed in a timely fashion.
 
5. Equivalent to an excellent consultant or expert in the area- this level rarely seen.
