Head Coach Survey

The current head coach evaluation form is fair and demonstrates an accurate reflection of a coach's performance throughout the season.

The current form is easy to read and interpret.

The number system of 1 through 5 is an effective way to rate a coach's performance for each of the criteria listed on the evaluation form.

A qualitative summative rating (example: accomplished) is preferred over a numerical rating (example: 1.00 to 5.00).
