Hello:
The Agency is working on a project to ensure that the Compass Program used to submit the Florida Hospital Uniform Reporting System (FHURS) data will be compatible with current and future versions of Windows and Excel. In order to make this project a success, we need your help. Please take a moment to answer a few questions about the particular operating system and Microsoft Office version currently used by your facility to complete the FHURS form.

Your answers will help us ensure that the system is compatible with the different systems used by various hospitals across the state.

This survey is being sent to more than 250 facilities in the state of Florida, should be filled out either by the person responsible for completing the Compass form or someone who has specific knowledge of your facility’s particular system/environment, and will take approximately 5 minutes to complete. Your participation in this survey is completely voluntary and responses will be coded and remain confidential.
