INDIVIDUAL STUDENT DATA ENTRY FORM
 
 
It is intended that the teacher in charge of the IRC Pilot at the school will complete this form, in consultation with other relevant staff, for each student who is participating or who has participated in the IRC Pilot.

The evaluation requires two phases of data collection. This form is to be used for this initial collection and a subsequent collection in 2014. By entering these data, the Coordinator is also fulfilling DEEWR monitoring requirements to account for program expenditure.

A separate form needs to be completed for each student. Once you have completed entering the data for one student, click 'Continue' to save the form and access another form for the next student.

The data collected does not identify students, although a data code for each student allows student data can be tracked from one phase of data collection to the other. The Evaluators are not to be provided with any link between the data code and the names of students.

The data to be collected include the student’s date of birth, their school Year, attendance records (explained and unexplained absences) for last year and for the two terms prior to commencing the program, and while participating in the program, NAPLAN bands, and progress towards attainment of Certificate 1 and 2.

By going to the bottom of any page and clicking on "Save page and Continue later" you can save the form and return to it at a later time. Your data for that student will be saved and you will be sent a link to allow you to access and complete the form for that student from the point where you left off.
