Dear Mentor,

Thank you very much for taking your time to provide this information. It will help us strengthen the Lead Academy programme and provide data to demonstrate the effects of mentoring on mentors and mentees. All the individual data from this survey will be kept anonymous in published reports. It is important that you answer all questions as honestly as you can. Remember that there are no right or wrong answers, so please avoid answering the questions based on what you think we want to hear. Only the survey coordinators will see how you answer the questions, and he or she will keep your answers confidential.
