My name is Anita Patel and I am conducting a research project, which is part of the requirement for my MBA in Executive Management at Royal Roads University. The research will consist of this survey and is foreseen to take 15 – 20 minutes to complete. The foreseen questions will refer to your understanding of lean management initiatives and how they can be integrated into the organization and allow all employees to develop innovative solutions to eliminate waste and enable employees to be cross-trained.

In addition to submitting my final report to Royal Roads University in partial fulfillment for a Master of Business Administration degree. An electronic copy, with no public access, will be held for a period of one year at Royal Roads University. Due to confidential company information the report will not be shared with future MBA learners.
The information you provide will be summarized, in anonymous format, in the body of the final report. At no time will any specific comments be attributed to you unless your specific agreement has been obtained beforehand. All documentation will be kept strictly confidential.
