Hello:
You are invited to participate in our Jazzercise Malaysia "Sweat to Smiles" campaign in aid of MAA Medicare Kidney Charity Fund survey. This survey is designed with the purpose of :
1) documenting the impact and success story of Jazzercise Malaysia in organising a major event since 10-10-10
2) identifying primary issues faced by participants concerning the event (from the time of confirmation of event to the completion of the event)
3) identifying key areas that can be improved on, in terms of event planning and event management
4) gathering constructive feedback from participants regarding the overall event (before, during and after). 

In this survey, approximately 50 people will be asked to complete a survey that asks questions about the event. It will take approximately 5 to 15 minutes, depending on your responses, to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions in order to improve on future events.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
