The purpose of the survey centers on mathematics instruction. This data will be used to help determine my course of action for my action research project. The expected duration of your participation in this survey is less than 5 minutes. Only the researcher will handle data. All data collected and recorded will be done so anonymously. The surveys and the data will be destroyed upon the completion of the study. The results of the study will be reported accurately regardless of results.
