You are invited to participate in my survey on Midlife Crisis, Approximately 100 people will be asked 16 questions about facing transitions and turning points during middle adulthood. It will take approximately 5 minutes to complete the questionnaire. Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. Your survey responses will be kept strictly confidential and data from this research will be reported only in the aggregate. Do NOT include your name. Please read the following scenario below before continuing on to the survey:

You wake up on the morning of your 45th birthday and realize that you are probably more than halfway through your life. You reflect on your accomplishments and lament over certain situations that you are not satisfied with. You start to think about how and if you are going to change your life. Maybe you've missed out on doing something extraordinary because of family or work obligations. You can’t reverse your life, so how are you going to face it going forward? Some people experience a “midlife crisis”, which is a transition or turning point when the course of events in life has changed. Studies indicate that it usually happens between the ages of 40-65 or midlife, and various transitions can trigger a midlife crisis. Recent studies show it can happen as early as 35 and as late as 70 years old.
Reflecting on the above scenario, please answer the following questions by circling the answer that best describes your situation. You do not have to answer any questions you aren’t comfortable with. Feel free to leave personal comments for any questions.
