Hello:
You are invited to participate in our Pathways to Wellness Clinic Staffing service needs survey for FY2013-2014. This particular survey is meant for executive leadership level personnel only. In this survey, approximately 7 executive level staff members will be asked to complete a survey that asks questions about our current staffing structure within each clinics and agency as a whole. Your feedback around how to adequately staff our agency next fiscal year in order to better serve our patients as well as meet our contractual requirements is extremely important to us. Our upcoming fiscal year begins on July 1, 2013 and our CEO will need to submit a final version of our FY2013-2014 budget by June 17, 2013. It will take approximately 20 minutes to complete the questionnaire.

Your participation in this survey is mandatory. It is very important for us to learn your opinions and make the necessary improvements to remain a key provider in this market.
