I am preparing annual reviews for my team members and would appreciate your feedback about their individual performance during the period of April 1, 2011 to present. Please provide details or examples when possible.

If the employee you are reviewing is a PM or EA, there are also four questions at the end of the survey that will be used to set goals and tailor individual development plans for the next review period. I will not receive your feedback from this section until after annual reviews are complete. Thank you for your insight as I prepare their evaluations and discuss their performance.

In an effort to better position the EPMO as a business-centric, entrepreneurial organization, the EPMO has identified four areas in which we would like to focus staff development activities in 2012.
