In this survey people will be asked about specific competencies which have been noted as development opportunities for pharmacist leaders. You will have a chance to rate each opportunity. It will take approximately 25 minutes to complete the questionnaire.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions so that we can better improve leadership development courses to meet your needs.

The following section of the survey will examine knowledge, skills, and abilities (KSA's) that have been established by the American Society of Health-System Pharmacists as critical to leadership. Our goal is to determine areas of opportunity for learning and career development within our system. With this in mind, we ask you to be as candid as possible on this survey.

We will ask you to rate two aspects of each KSA listed;
1)The importance of each in determining success in your current or future role
2)Your current proficiency level for that KSA.
